Responsibilities

• Conduct comprehensive in-hospital case management for assigned patients, emphasizing discharge planning and care coordination
• Collaborate with the multidisciplinary team to develop and implement individualized care plans
• Facilitate safe patient transitions across settings and coordinate post-acute services and community resources
• Monitor patient progress, identify barriers to throughput, and ensure regulatory and payer requirements are met
• Document care decisions in the electronic medical record and participate in floating to sister campuses as needed

Requirements

• Current RN license with Nursys verification on profile
• BLS certification
• Minimum 2 years of Case Management experience
• Education: Accredited nursing degree; or 5 years of case management experience as an alternative
• Ability to float to other campuses as needed
