Responsibilities

• Perform comprehensive PT evaluations and establish individualized treatment plans for residents
• Deliver evidence-based physical therapy interventions focused on mobility, strength, balance, and functional independence
• Monitor progress, adjust treatment plans, and document outcomes for each resident
• Collaborate with the interdisciplinary care team to coordinate care and support discharge planning
• Educate residents and families on exercises, safety, and home programs
• Comply with facility policies, regulations, and charting requirements while maintaining high standards of patient care

Requirements

• Active Physical Therapist license in New Jersey (or eligibility for licensure in NJ)
• Graduation from an accredited physical therapy program (DPT or equivalent)
• Experience in skilled nursing facility or geriatric rehabilitation preferred
• Ability to work 35 hours per week in a long-term care setting
• Strong communication and documentation skills; ability to collaborate with a multidisciplinary team
