Responsibilities

• Assess and treat residents with physical therapy needs to improve mobility, strength, balance, and functional independence
• Develop and implement individualized plans of care in collaboration with the interdisciplinary team
• Administer evidence-based therapies and modalities appropriate for geriatric rehabilitation
• Document evaluated findings, treatment plans, progress, and discharge summaries in the medical record
• Monitor patient response to treatment and adjust plans as needed
• Educate residents and caregivers on home exercise programs, safety, and fall-prevention strategies
• Ensure patient safety, infection control, and adherence to facility policies and regulations

Requirements

• Active physical therapy license in good standing (state-specific licensure; Mississippi license preferred)
• Doctoral degree in Physical Therapy (DPT) or equivalent
• Experience in skilled nursing facility or geriatric rehabilitation preferred
• Ability to work in a travel assignment and adapt to a SNF setting
• Strong communication and documentation skills
