Responsibilities

• Document evaluations, treatment sessions, and progress notes in a timely, compliant manner.
• Collaborate with nursing staff and other rehabilitation professionals to coordinate care and support discharge planning.
• Educate residents and families on exercises, safety strategies, and home exercise programs.
• Maintain safety, infection control, and quality standards during all therapy sessions.

Requirements

• Active Physical Therapist license in New Hampshire or eligibility for NH licensure.
• Ability to work effectively in a skilled nursing care environment and collaborate with an interdisciplinary team.
• Strong written and verbal communication skills for accurate documentation and resident education.
