Role:

Psychiatry Nurse Practitioner providing compassionate mental health care to seniors across long-term care, assisted living, and community settings in a hybrid on-site/telehealth model.

Key Responsibilities

• Conduct comprehensive psychiatric evaluations for older adults.
• Assess mental health needs and develop individualized treatment plans.
• Manage and monitor psychiatric medications, ensuring safe, effective care.
• Deliver care through both on-site and telehealth visits.
• Collaborate with an interdisciplinary team and partner facilities to support emotional well-being, safety, and overall quality of life.
