Primary Care Nurse Practitioner or Physician Assistant

Role: Nurse Practitioner or Physician Assistant to deliver patient-centered primary care in a collaborative, physician-led team.

Key Responsibilities

• Perform comprehensive patient assessments and develop individualized care plans.
• Diagnose and manage acute and chronic conditions.
• Order, interpret, and follow up on diagnostic tests.
• Prescribe medications and manage treatment plans in accordance with clinical standards.
• Coordinate care across specialties and with behavioral health and ancillary services.
• Provide preventive care, health education, and support for individuals and families.
• Communicate effectively with patients to build strong, ongoing relationships.

Qualifications & Skills

• Licensed NP or PA with training to provide primary care.
• Strong clinical judgment for assessment, diagnosis, and ongoing condition management.
• Competence in diagnostic test ordering/interpretation and medication management.
• Excellent patient communication and health coaching abilities.
• Ability to collaborate within multidisciplinary teams to ensure coordinated, seamless care.
