Occupational Therapist (Home Health)

Role:

Provide evidence-based occupational therapy to older adults in their homes and senior living communities.

Key Responsibilities

• Conduct comprehensive evaluations to assess functional needs and safety concerns.
• Develop individualized treatment plans tailored to patient goals.
• Deliver proactive geriatric rehabilitation to promote independence and quality of life.
• Collaborate with an interdisciplinary team to support patient-centered care.

Qualifications & Skills

• Licensure/credential as an Occupational Therapist.
• Strong understanding of geriatric care and functional rehabilitation.
