Your Role as an RN Nurse Assessor:

• Conduct in-home patient assessments & Telehealth
• Complete UAS-NY assessments accurately
• Evaluate medical, functional & care needs
• Coordinate with interdisciplinary healthcare teams
• Help patients access essential long-term care services

Requirements:

• Active New York RN License
• 2 Years of Experience in Home Health, Assessments, or Long-Term Care
• (either it is RN, LPN or HHA)
