Your Role as an RN Nurse Assessor:

• Conduct in-home patient assessments
• Complete UAS-NY assessments accurately, (Training will be provided by Client)
• Help patients access essential long-term care services

Requirements:

• Active New York RN License
• Experience in Home Health, Assessments, or Long-Term Care
• Strong communication & clinical decision-making skills
• Comfortable using Microsoft Office & technology
• Ability to travel within assigned region
