Job Summary

In Home Rehab Group is seeking a talented and compassionate Physical Therapist - Part-time (Home Health) to join our dynamic team in Broward County, Florida. As a Physical Therapist with our group, you will have the opportunity to work in the comfort of patients' own homes, providing personalized outpatient physical therapy and occupational therapy services.

Responsibilities

• Evaluate and treat patients with a variety of conditions, including but not limited to orthopedic, neurological, and cardiovascular disorders
• Develop and implement individualized treatment plans aimed at improving patients' functional abilities and reducing pain
• Conduct initial and ongoing assessments, and provide progress reports to patients, their families, and the referring physician
• Collaborate with other healthcare professionals, including occupational therapists and medical social workers, to ensure comprehensive care
• Maintain accurate and up-to-date records of patient treatment, including documentation of patient interactions, treatment plans, and progress notes
• Participate in interdisciplinary team conferences and staff meetings to discuss patient care and share best practices

Requirements

• DPT degree or equivalent from an accredited physical therapy program
• Current licensure to practice physical therapy in the state of Florida
• Ability to work independently and as part of a team
• Excellent communication and interpersonal skills
• Strong organizational and time management skills
• Proficiency in electronic medical records (EMRs)
