Physical Therapist Home Health (PT HH) The healthcare provider is seeking a licensed Physical Therapist to provide home health services. The role involves delivering patient-centered care through various therapeutic interventions to adult and geriatric patients within a home health setting. The position offers flexible scheduling options, including 10-hour shifts or a four-day workweek.

Job Details:

• Certifications, Education, and Experience Requirements: CA Physical Therapist license, at least 1 year of experience as a home health Physical Therapist, valid drivers license and auto insurance, BLS certification preferred.
• Schedule: 8-hour shifts, 40 hours per week, starting 11/09/2026, ending 02/08/2027.
• Responsibilities: Providing therapeutic assessments, both clinical and functional, developing and implementing patient care plans, utilizing adaptive devices and therapeutic techniques, educating patients and families, and documenting treatment progress.
• Additional Information: Open to flexible scheduling including 10-hour shifts or 4-day workweeks. Candidates should have experience with charting systems, patient assessment, and delivering care to adult and geriatric populations.
