BrightStar Care of TylerLongview is seeking a skilled, compassionate Physical Therapist to join our nurse-led care team. Whether you're looking to supplement your current schedule or work on a flexible PRN basis, you can set your availability and accept visits that fit your life.
The Opportunity
- Pay: $48-$55 per hour, based on experience
- Schedule: PRN-as-needed home visits that fit your availability
- Territory: Longview, Kilgore, Gladewater, Henderson, and Lake Cherokee
- Reports to: Director of Nursing
- Support: Collaborate with our RN-led team while working independently in clients' homes
What You'll Do
- Conduct in-home evaluations, re-evaluations, and discharge assessments.
- Develop individualized treatment plans with measurable goals in coordination with the physician and Director of Nursing.
- Provide therapeutic exercise, gait and balance training, transfer training, and fall-prevention guidance.
- Recommend equipment and adaptive devices, and teach clients how to use them safely.
- Educate clients, families, and caregivers on home exercises, safe handling, and home safety.
- Complete accurate visit documentation in BrightStar's ABS clinical system within 24 hours.
- Promptly report changes in condition to the RN case manager and physician.
- Follow infection-control, HIPAA, and agency policies, and participate in case conferences as needed.
What You'll Bring
- Doctor or Master of Physical Therapy from an accredited program
- Current, unrestricted Texas PT license or valid PT Compact privilege to practice in Texas
- Current CPR/BLS certification
- Strong communication skills and confidence working independently
- Valid Texas driver's license, reliable transportation, and current auto insurance
- Ability to pass a background check, drug screen, and TB screening
One year or more of PT experience is preferred. Home health or geriatric experience is a plus.
This role requires driving between client homes throughout Gregg and Rusk Counties, assisting with up to 50 lbs, and kneeling, bending, and standing for extended periods.
