BrightStar Care of TylerLongview is seeking a skilled, compassionate Physical Therapist to provide personalized rehabilitation in clients' homes. Set your availability, accept visits that fit your schedule, and bring your expertise to a supportive, nurse-led care team.

Tyler, Whitehouse, Bullard, Flint, Lindale, Winona, Chapel Hill, Lake Tyler, and Lake Palestine.
What You'll Do
- Complete in-home evaluations, re-evaluations, and discharge assessments.
- Develop individualized treatment plans with measurable goals in coordination with the physician and Director of Nursing.
- Provide therapeutic exercise, gait and balance training, transfer training, and fall-prevention guidance.
- Recommend durable medical equipment and adaptive devices, and teach clients how to use them safely.
- Educate clients, families, and caregivers on home exercises, safe handling techniques, and home safety.
- Document each visit accurately in BrightStar's ABS clinical system within 24 hours.
- Promptly communicate changes in condition to the RN case manager and physician.
- Follow infection-control, HIPAA, and agency policies, and participate in case conferences as needed.
What You'll Bring
- Doctor or Master of Physical Therapy from an accredited program
- Current, unrestricted Texas PT license or valid PT Compact privilege to practice in Texas
- Current CPR/BLS certification
- Strong communication skills and the ability to work independently
- Valid Texas driver's license, reliable transportation, and current auto insurance
- Ability to pass a background check, drug screen, and TB screening
One year or more of PT experience is preferred. Home health or geriatric experience is a plus.
Physical requirements
- Ability to lift or assist up to 50 lbs, kneel, bend, stand for extended periods, and drive between client homes throughout Smith County.
