Home Health Physical Therapist - Full Time

Position Overview

Seeking a

Full-Time Physical Therapist (PT)

to join a home health care team, enabling patients to recover at home.

Key Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, coordination, and functional status.
• Administer, evaluate, and interpret tests to set goals and measure outcomes.
• Provide written evaluations and develop personalized plans of care, documenting progress and modifying treatments as needed.
• Deliver therapy services aligned with physician orders, ensuring compliance.
• Participate in discharge planning, educating patients and caregivers on home exercise programs.
• Coordinate with healthcare teams, physicians, and families, maintaining clear communication.
• Document effectively to prevent claim denials and ensure compliance with policies and regulations.
• Supervise other personnel such as PTAs and HHAs as applicable.
• Complete OASIS assessments where permitted by regulation.

Qualifications & Skills

• Degree in Physical Therapy from an accredited program.
• Current, unrestricted state license as a PT.
• Valid CPR certification.
• Preferred: strong organization, communication, attention to detail, time management, and experience in home health therapy and OASIS documentation.
