Job Summary

We are seeking a skilled, compassionate, and dedicated Physical Therapist (PT) to provide high-quality patient care and rehabilitation services. The ideal candidate will assess patients’ physical conditions, develop individualized treatment plans, and implement therapeutic interventions to improve mobility, reduce pain, restore function, and enhance overall quality of life.

The Physical Therapist will collaborate with physicians, nurses, and other healthcare professionals to ensure effective, patient-centered care while maintaining accurate clinical documentation and complying with established healthcare standards and regulations.

Responsibilities

  • Evaluate patients’ physical conditions, mobility, strength, balance, and functional limitations.
  • Develop and implement individualized treatment plans based on patients’ needs and rehabilitation goals.
  • Provide therapeutic exercises and physical therapy interventions to improve strength, flexibility, coordination, and range of motion.
  • Assist patients in recovering from injuries, surgeries, illnesses, and physical disabilities.
  • Apply appropriate treatment techniques to reduce pain, improve mobility, and restore functional independence.
  • Monitor patients’ progress and modify treatment plans based on their response to therapy.
  • Educate patients and their families on exercises, injury prevention, posture, body mechanics, and home exercise programs.
  • Assist patients with gait training, balance exercises, transfers, and mobility training as appropriate.
  • Collaborate with physicians, nurses, occupational therapists, and other healthcare professionals to coordinate patient care.
  • Maintain accurate and timely documentation of patient evaluations, treatment sessions, progress notes, and discharge summaries.

Requirements

  • Active New Hampshire PT license required.
  • Current BLS certification required.
  • Candidates with 6+ months of CNA experience are welcome.

License/Certification:

  • NH-State License (Required)
