Job Summary

Join our collaberative healthcare team as a Physical Therapist or Physical Therapist Assistant, where your expertise will directly impact patient recovery and quality of life while having fun at the same time!

In this role, you will deliver personalized rehabilitative care, develop and implement treatment plans, and collaborate with a multidisciplinary team to ensure optimal patient outcomes. Duties

  • Assess patients’ physical conditions through detailed evaluations, including physiology and anatomy knowledge, to inform treatment strategies.
  • Develop, implement, and adjust individualized care plans tailored to each patient’s needs, including discharge planning and goal setting.
  • Provide hands-on therapy such as therapeutic exercises, trigger point therapy, and mobility training to facilitate patient rehabilitation.
  • Document all patient interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards.
  • Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate care and optimize treatment outcomes.
  • Manage patient care across various settings such as outpatient clinics, inpatient hospital units, home health visits, and community care environments.
  • Assist in discharge planning by evaluating progress and recommending ongoing therapy or community resources for continued recovery.

Skills

  • Proven experience practicing in a physical therapy role within outpatient clinics, hospitals, or home health settings.
  • Strong knowledge of physiology, anatomy, orthopedics, neurology, pediatrics, geriatrics, and related medical terminology.
  • Skilled in developing comprehensive treatment plans that incorporate physiotherapy techniques like trigger point therapy and patient rehabilitation strategies.
  • Familiarity with outpatient rehabilitation center operations and inpatient hospital workflows to ensure seamless patient care transitions.
  • Proficiency in electronic health records (EHR) management and EMR systems for accurate documentation of treatment plans and progress notes.
  • Excellent client communication skills to educate patients on their conditions and motivate active participation in their recovery process.
  • Ability to work collaboratively with healthcare teams on discharge planning, acute care management, and community-based services.
  • Ability to have flexibility with schedule, working with trusted and genuine employees
