Registered Nurse Home Health Visits

Vertus HealthCare, a Medicare-certified home health agency serving patients throughout Palm Beach County, is seeking experienced Registered Nurses (RNs) to provide quality skilled nursing care to patients in the comfort of their homes. As a Registered Nurse Home Health Visits, you will play a vital role in delivering exceptional care, promoting independence, and improving outcomes for our patients.

Responsibilities:

• Provide skilled nursing visits based on the patient's plan of care
• Complete OASIS, SOC, ROC, Recertification, and Discharge visits as assigned
• Assess patient needs and coordinate care with the clinical team
• Complete timely and accurate clinical documentation
• Communicate with physicians, patients, families, and the healthcare team

Qualifications:

• Current Florida RN license
• Home Health experience preferred
• OASIS experience preferred
• Strong clinical assessment and documentation skills
• Reliable transportation
