PRN Physical Therapist (PT) – Home Health | $75+/Hour

Job Type: PRN / Per Diem

Location: Chicagoland, IL | Field-based / On the road

Add Flexible Home Health Visits Around Your Current Schedule

We are hiring multiple experienced Home Health Physical Therapists who want to earn additional income by adding PRN patient visits around their current schedule.

Starting pay is $75/hour.

We currently have opportunities in two territories:

  • South / Southwest Suburbs: approximately 10–12 visits per week currently available
  • Lake County / Far North Suburbs: approximately 10–15 visits per week of potential coverage, depending on census and referrals

This is a PRN Home Health role, not a full-time position. It is designed for experienced home health PTs who want to keep their current job or caseload while adding flexible local visits for supplemental income.

Previous professional Home Health PT experience and experience treating orthopedic/post-surgical patients are required.

You do not need to cover all of Chicagoland. We want to align patients with the territory you already live in, work in, or regularly cover.

Current patient needs are concentrated around:

  • Joliet, Plainfield, Orland Park, Lockport, Lemont, Homer Glen, New Lenox, Mokena, Frankfort, Tinley Park, Romeoville, Bolingbrook, Shorewood, and surrounding communities.

Approximately 10–12 visits per week are currently available in this territory.

  • Lake County / Far North Suburbs

Current coverage needs include:

Gurnee, Zion, Grayslake, Antioch, Waukegan, North Chicago, Beach Park, Round Lake, Lindenhurst, Lake Villa, Fox Lake, Libertyville, Mundelein, Vernon Hills, and surrounding communities.

There is potential for approximately 10–15 visits per week, depending on patient census and referrals.

You are not expected to cover the entire territory. Patient assignments are aligned with your location, availability, and reasonable travel radius whenever possible.

Required Qualifications

To be considered for this position, you must have:

  • Active Illinois Physical Therapist (PT) license
  • Previous professional Home Health Physical Therapy experience
  • Experience treating orthopedic and post-surgical patients
  • Experience completing skilled PT evaluations, reassessments, and treatments in patients’ homes
  • Strong clinical assessment and treatment planning skills
  • Ability to work independently in a field-based home health environment
  • Strong clinical documentation and time-management skills
  • Valid driver’s license and reliable transportation
  • Ability to travel locally between patient homes

Experience with Start of Care (SOC), admissions, OASIS, and Homecare Homebase (HCHB) is strongly preferred.

What You’ll Do

  • Provide skilled physical therapy services in patients’ homes
  • Treat orthopedic, post-surgical, post-acute, and other home health patients
  • Complete PT evaluations, reassessments, treatments, and other skilled visits
  • Develop individualized plans of care focused on mobility, strength, balance, safety, and functional independence
  • Complete Start of Care visits and admissions as needed, including PT-only cases
  • Monitor patient progress and adjust treatment plans as appropriate
  • Educate patients and caregivers on mobility, fall prevention, safety, home exercise programs, and recovery goals
  • Complete OASIS and other required home health documentation as applicable
  • Document patient care accurately and on time
  • Coordinate care with nursing, physicians, other therapists, patients, and caregivers
  • Manage your own daily patient schedule and route

Schedule

This is a PRN / Per Diem position.

Patient volume varies based on referrals, census, territory, and your availability. You can discuss your preferred number of visits, days, and geographic coverage during the interview process.

The position can be worked alongside an existing full-time, part-time, or PRN role.

Who We’re Looking For

The ideal candidate is an experienced Home Health Physical Therapist who already understands the independence and documentation requirements of working in patients’ homes.

You should be comfortable treating orthopedic and post-surgical patients, managing your own schedule and route, communicating with the interdisciplinary team, and providing high-quality care with minimal day-to-day supervision.

Previous Home Health PT experience, orthopedic/post-surgical experience, and an active Illinois PT license are required.
