Key Responsibilities

• Evaluate residents and develop individualized treatment plans.
• Provide physical therapy services to improve mobility, strength, balance, endurance, and functional independence.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with interdisciplinary teams to support optimal patient outcomes.
• Educate patients, families, and caregivers on exercises, safety, and treatment goals.
• Maintain accurate and timely clinical documentation.
• Participate in care planning and discharge planning activities.

Qualifications

• Active California Physical Therapist License.
• Graduate of an accredited Physical Therapy program.
• SNF experience preferred, but not required.
• Strong clinical, communication, and documentation skills.
• Ability to work effectively in a patient-centered, team-oriented environment.
