Key Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans.
• Provide skilled interventions to improve daily living skills, coordination, strength, and functional independence.
• Monitor patient progress and modify treatment plans as needed.
• Collaborate with interdisciplinary teams to achieve optimal patient outcomes.
• Educate patients, families, and caregivers on adaptive techniques, exercises, and safety precautions.
• Maintain accurate and timely clinical documentation.
• Participate in care planning and discharge planning activities.

Qualifications

• Active California Occupational Therapist license.
• Graduate of an accredited Occupational Therapy program.
• SNF experience preferred, but not required.
• Strong clinical, communication, and documentation skills.
• Ability to work effectively in a patient-centered, team-oriented environment.
