Duties
- Conduct comprehensive assessments of patients’ physical condition in a home setting, including evaluating mobility, strength, balance, and functional abilities.
- Develop and implement individualized care plans tailored to each patient’s specific needs, goals, and medical conditions.
- Administer therapeutic interventions such as manual therapy, trigger point therapy, gait training, and therapeutic exercises to facilitate recovery.
- Manage discharge planning processes to ensure smooth transitions from hospital or outpatient settings to home care.
- Document all patient interactions accurately in electronic health records (EHR) systems following clinical documentation standards.
- Collaborate closely with healthcare professionals—including physicians, nurses, occupational therapists—to coordinate care plans and optimize patient outcomes.
- Educate patients and their families on treatment techniques, safety measures, and home exercise programs to promote ongoing rehabilitation.
- Perform ongoing evaluations of treatment effectiveness and adjust plans accordingly to meet evolving patient needs.
Experience
- Proven experience practicing in a physical therapy role within home health or outpatient clinics.
- Strong background in outpatient rehabilitation center environments with exposure to orthopedic, neurology, pediatrics, and geriatrics populations.
- Demonstrated ability to work effectively with geriatric patients and those requiring complex discharge planning.
- Familiarity with inpatient hospital settings or inpatient experience is highly desirable.
- Knowledge of physiology, anatomy, medical terminology, and clinical documentation standards essential for accurate record keeping.
- Experience managing electronic health records (EHR) or electronic medical records (EMR) systems efficiently.
- Prior involvement in outpatient clinic settings or outpatient rehabilitation centers that emphasize patient care excellence.
- Ability to communicate clearly with clients and healthcare teams about treatment plans and progress updates.
