Job description

About this role

Evaluates patients’ functional abilities and limitations through review of medical records, patient interviews, clinical observation, and standardized assessments.

Ø Assesses physical, cognitive, sensory, and psychosocial factors affecting patients’ ability to perform daily activities.

Ø Develops individualized treatment plans with measurable goals based on evaluation findings, patient priorities, clinical judgment, and applicable provider orders.

Ø Provides occupational therapy interventions within scope of practice, training, and demonstrated competency, including therapeutic activities, therapeutic exercise, self-care training, and strategies to improve participation in daily routines.

Ø Instructs patients in adaptive techniques, activity modification, joint protection, energy conservation, body mechanics, and safe use of assistive equipment.

Ø Recommends adaptive equipment and environmental modifications to support safety and independence at home, at work, and in the community.

Ø Monitors patients’ responses to treatment, reassesses progress, and modifies treatment plans as needed.

Ø Coordinates care with physicians, APPs, physical therapists, and other members of the healthcare team.

Ø Plans for discharge, including home program recommendations, caregiver training, and coordination of additional services or resources.

Salary:Hiring range, based on experience and credentials: $88,524.80-132,787.20 per year

Workdays: This role will be primarily located at the East-side rehab clinic in Northeast Portland. Occasional coverage will be required on an as-needed basis at The Oregon Clinic Creekside Pavilion in Southwest Beaverton. Typical hours are Monday-Friday (Shift schedules are flexible, with ending times of either 5:40 PM or 6:00 PM, and based on the candidate's preference for an 8-, 9-, or 10-hour workday).

Qualifications that support success in this role are based on education, experience and values including:

Required:

  • Current Oregon state license as an Occupational Therapist
  • Current OTR certification through NBCOT.
  • 2+ years’ experience as an Occupational Therapist.
  • Current CPR/BLS within 60 days of hire. ACLS may be required by specialty.

Preferred:

  • 5+ years’ experience as an Occupational Therapist.
  • Certification as a Hand Therapist (CHT).
