Home Health RN Case Manager

Responsibilities

• Provide direct patient care in home settings, assessing patient needs and developing individualized care plans.
• Administer medications and treatments as prescribed by physicians.
• Educate patients and families on health management and care procedures.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.
• Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
• Maintain accurate medical records and documentation of patient interactions.

Requirements

• Current Registered Nurse (RN) license in the state of Texas.
• 1 Year Home Health experience
• Proven experience in patient care, preferably in home health or hospice settings.
• Strong knowledge of geriatric care practices and principles.
• Excellent communication skills for effective interaction with patients and families.
• Ability to work independently while managing multiple patient cases.
• Kinnser experience preferred

Job Type: Full-time

Pay: From $80,000.00 per year
