Home Health Physical Therapist PRN

Your Role

• Conduct thorough assessments and develop personalized care plans for home health patients.
• Implement therapies to improve mobility, strength, and overall function.
• Educate patients and families on techniques for maintaining progress and safety.
• Document treatments and coordinate with the multidisciplinary care team.
• Develop care plans using a variety of treatment techniques

What We’re Looking For

• Current licensure as a Physical Therapist in Texas.
• Strong interpersonal skills and a passion for patient care.
• Experience in home health required.
• Experience with Kinnser/WellSky (preferred but not required).
• Ability to work independently and adapt to varied patient needs.
