Sierra Nevada Home Health is seeking a compassionate, motivated, and dependable Physical Therapist (PT) to join our growing home health team in the Fernley, Nevada area!

We are looking for a skilled therapist who enjoys providing personalized, one-on-one patient care in the comfort of patients' homes. Job Responsibilities

• Perform comprehensive physical therapy evaluations in patients' homes.
• Develop individualized treatment plans based on patient needs, functional abilities, and physician orders.
• Provide skilled physical therapy services to improve mobility, strength, balance, endurance, and overall functional independence.
• Assess gait, transfers, fall risks, and home safety.
• Establish individualized home exercise programs and educate patients and caregivers.
• Evaluate the need for assistive devices and durable medical equipment.
• Collaborate with nurses, occupational therapists, physicians, and other members of the interdisciplinary team.
• Supervise Physical Therapist Assistants (PTAs) as assigned and in accordance with applicable regulations.
• Complete timely and accurate clinical documentation in accordance with Medicare guidelines and agency policies.
• Participate in care coordination, reassessments, recertifications, and discharge planning.

Qualifications

• Current and unrestricted Physical Therapist license in the State of Nevada.
• Graduate of an accredited Physical Therapy program.
• Home health experience preferred, but not required.
• Knowledge of Medicare home health regulations and documentation requirements preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and manage a flexible patient schedule.
• Valid driver's license, reliable transportation, and current auto insurance.
• Current CPR certification.

Pay: $123,000.00 - $148,000.00 per year
