Sierra Nevada Home Health is seeking a compassionate, motivated, and dependable Occupational Therapist (OT) to join our growing home health team!

We are looking for an experienced therapist who enjoys providing personalized, one-on-one patient care in the comfort of patients' homes. This is an excellent opportunity for an OT seeking a flexible schedule, independence, and the ability to make a meaningful difference in patients' lives.

Job Responsibilities

• Perform comprehensive occupational therapy evaluations in patients' homes.
• Develop individualized treatment plans based on patient needs, functional abilities, and physician orders.
• Provide skilled occupational therapy services to improve independence, safety, and quality of life.
• Assess and address activities of daily living (ADLs), mobility, home safety, and adaptive equipment needs.
• Educate patients, families, and caregivers on therapeutic techniques and home exercise programs.
• Collaborate with nurses, physical therapists, physicians, and other members of the interdisciplinary team.
• Complete timely and accurate clinical documentation in accordance with Medicare guidelines and agency policies.
• Participate in care coordination, reassessments, and discharge planning.

Qualifications

• Current and unrestricted Occupational Therapist license in the State of Nevada.
• Graduate of an accredited Occupational Therapy program.
• Home health experience preferred.
• Knowledge of Medicare home health regulations and documentation requirements preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to work independently and manage a flexible patient schedule.
• Valid driver's license, reliable transportation, and current auto insurance.
• Current CPR certification.
