Outpatient Orthopedic Physical Therapist

Sforzo | Dillingham | Stewart Orthopedics + Sports Medicine is seeking a licensed Physical Therapist to join our physician-owned, patient-centered orthopedic practice. This position is designed for a therapist who values individualized care, manageable patient volumes, and the opportunity to work closely with orthopedic physicians and other rehabilitation professionals.

Our model allows therapists to focus on delivering high-quality care without group therapy sessions or managing multiple patients at the same time.

Key Responsibilities

• Evaluate patients and develop individualized plans of care based on their orthopedic conditions and functional goals.
• Provide one-on-one physical therapy treatments in accordance with established clinical standards.
• Work collaboratively with our orthopedic physicians, advanced practice providers, and rehabilitation team.
• Monitor patient progress and adjust treatment plans as appropriate.
• Educate patients on their conditions, home exercise programs, and strategies for continued recovery.
• Maintain timely and accurate documentation within the electronic health record.
• Help provide an attentive, professional, and supportive experience throughout each patient’s rehabilitation journey.

Requirements

• Graduate of a CAPTE-accredited Physical Therapy program
• Current or eligible Florida Physical Therapist license
• Current BLS/CPR certification
• Strong communication, organization, and documentation skills
• Commitment to individualized, patient-centered orthopedic care
• Working knowledge of Strata and ModMed EMA or other EHR applications is preferred
