About this role
The NP is responsible for practicing the nursing profession ethically, dutifully, and according to and within the Scope and Standards of Practice of the Texas Board of Nursing. The NP is responsible for Provider-level care coordination, as outlined in the Texas Independence Health Plan (ISNP) Model of Care, for all ISNP Members.
The NP is generally expected to be in his/her facilities during customary working hours (approximately 8:00am – 5:00pm) to ensure that these facilities have adequate Provider-Level coverage to meet patient/resident/member health care needs while also allowing the NP to have adequate time for documentation, and rotational weekday/weekend on-call responsibility.
Nurse Practitioners Services of Texas is dedicated to delivering proactive, patient-centered care to nursing home residents enrolled in the Texas Independent Health Plan. Our clinical model focuses on early identification of changes in condition, comprehensive health management, and increased on-site provider availability to reduce avoidable hospitalizations and emergency visits.
Our Nurse Practitioners establish individualized care goals, coordinate with families and facility staff, and guide advance care planning discussions. RN Care Coordinators support continuity of care, track quality measures, and partner closely with our providers to ensure exceptional outcomes.
At Nurse Practitioners Services of Texas, you’ll join a collaborative team that values compassion, clinical excellence, and a shared commitment to improving outcomes for a vulnerable population.
