We are seeking a compassionate, experienced, and clinically skilled Registered Nurse (RN) to join our Home Health team serving patients throughout the South Bay area.

We are looking for a dependable nurse with strong clinical judgment, excellent assessment skills, and the ability to independently manage patient care in a home health setting.

Bilingual Spanish-speaking or Vietnamese-speaking nurses are highly encouraged to apply!

Key Responsibilities

Perform comprehensive nursing assessments and evaluate patients' clinical conditions.

* Provide skilled nursing care in patients' homes. * Identify changes in patient conditions and communicate concerns promptly. * Develop, implement, and update individualized patient care plans. * Perform wound care, medication management, and other skilled nursing procedures. * Coordinate patient care with physicians, caregivers, and the interdisciplinary team. * Complete accurate and timely clinical documentation. * Educate patients and families regarding medications, disease management, and treatment plans. *

Qualifications

Active, unrestricted California Registered Nurse (RN) license.

* Strong clinical assessment and critical-thinking skills. * Experience in Home Health, Hospice, Acute Care, or a related clinical setting preferred. * Ability to work independently and make sound clinical decisions. * Excellent communication, organizational, and documentation skills. * Knowledge of OASIS documentation and Home Health regulations is a plus. * Bilingual Spanish or Vietnamese preferred. * Reliable transportation and willingness to travel throughout the South Bay area. *
