Job Overview

Responsibilities
- Conduct comprehensive patient evaluations to determine physical therapy needs, including assessments of physiology, anatomy, and medical history.
- Develop and implement tailored treatment plans utilizing evidence-based practices such as physiotherapy techniques, trigger point therapy, and orthopedics.
- Manage patient progress through ongoing assessment and adjustment of care plans to ensure optimal recovery outcomes.
- Collaborate closely with healthcare professionals such as physicians, nurses, and occupational therapists to coordinate care and discharge planning.
- Document all patient interactions accurately in electronic health records (EHR) systems adhering to clinical documentation standards.
- Educate patients and their families on treatment procedures, home exercises, and community resources to support ongoing rehabilitation.
- Assist with discharge planning by evaluating patient needs for outpatient or home health services and ensuring smooth transitions between care settings.
Requirements
- Valid state licensure or certification as a licensed Physical Therapist.
- Proven experience practicing in a physical therapy role within outpatient clinics, inpatient hospitals, or home health environments.
- Strong knowledge of physiology, anatomy, neurology, orthopedics, pediatrics, geriatrics, and geriatrics-specific care strategies.
- Familiarity with medical terminology and clinical documentation standards necessary for accurate record keeping.
- Experience working with diverse patient populations including geriatric patients, pediatric cases, and those requiring neurological rehabilitation.
- Proficiency in electronic health records (EHR) or electronic medical records (EMR) systems for documentation and treatment tracking.
- Ability to develop effective treatment plans that incorporate discharge planning and patient rehabilitation goals.
- Excellent client communication skills to educate patients about their care plans and foster positive therapeutic relationships.
