Contract Length: 13 Weeks (Potential for an extension)
Pay Range:
$60.00 to $63.00/hour*
Mileage Reimbursement Rate

*Pay offered is based on experience, expertise, credentialing, and education.
Duties:
- Conduct comprehensive clinical assessments to identify patient needs and determine appropriate occupational therapy interventions.
- Develop, document, and regularly update individualized
Plans of Care (POC)
, progress notes, physician orders, and other required clinical documentation.
- Provide therapeutic interventions to improve
ROM, strength, coordination, functional mobility, safety, and activities of daily living (ADLs)
.
- Evaluate occupational performance across self-care, work, leisure, and rest activities, including physical, sensory-motor, cognitive, and psychosocial factors.
- Recommend, fabricate, and maintain adaptive equipment and hand splints; educate patients on appropriate use and safety.
- Collaborate with physicians, caregivers, and interdisciplinary healthcare professionals to coordinate patient care and communicate patient progress.
- Follow infection control practices and universal precautions to maintain a safe patient-care environment.
- Maintain accurate patient records, analyze clinical data, and ensure timely completion of documentation.
- Provide patient and caregiver education to support treatment goals, functional independence, and continuity of care.
- Perform basic cleaning, setup, troubleshooting, and maintenance of occupational therapy equipment.
- Maintain compliance with applicable payer, regulatory, licensing, and organizational requirements.
Requirements:
- Current state Occupational Therapist (OT) license
- Required.
- Current CPR certification
- Required.
- Valid driver's license
- Required.
- Reliable transportation and current auto insurance
- Required.
- Minimum
2 years of Occupational Therapist experience
in an acute care or rehabilitation setting - Required.
- Graduate of an accredited Occupational Therapy program with a
Bachelor of Science degree in Occupational Therapy
.
- Experience with
Homecare Homebase (HCHB)
is required/preferred.
- Strong clinical assessment, documentation, communication, and organizational skills.
