PARC Home CARE is seeking Physical Therapists looking for autonomy and variety to join our therapy team and help clients throughout the Tri-County area! Design functional treatment plans for clients in their home environments, building interpersonal connections with those you care for, by helping clients achieve their goals to remain healthy at home.

Physical Therapist / DPT Qualifications

Possess a current PT / Physical Therapy license to practice in the State of Michigan

Experience in home care setting, namely OASIS documentation

Demonstrated ability to work autonomously

Valid Driver's License and reliable transportation

Physical Therapist / DPT Overview:

Provide Physical Therapy / PT services to our clients in their homes

Admissions / SOC ("Starts of Care"), evaluations, initial and ongoing assessments

Develop and revise patient's plan of care in collaboration with other clinicians

Plan, evaluate, and treat assigned clients, then evaluate outcomes

Pay: $50.00 - $120.00 per hour
