Nicole Reynolds, Occupational Therapist is seeking an Occupational Therapist with adult neurological rehabilitation experience to provide outpatient OT in patients’ homes on a per diem, as-needed basis.

This opportunity offers flexible scheduling within a small, therapist-owned practice. Visit availability varies based on referrals and patient needs, with no guaranteed minimum caseload.

Compensation

• Treatment visits: $60–$65 per completed visit
• Evaluations: $75–$80 per completed evaluation
• Rates based on relevant experience.

About the role

Our mobile practice serves adults with neurological conditions, including Parkinson’s disease and stroke. Treatment emphasizes independence in daily activities, functional mobility, upper-extremity function, coordination, and participation in meaningful routines.

Responsibilities

• Provide individualized OT evaluations and treatment.
• Develop functional, measurable goals and plans of care.
• Complete accurate, timely documentation in Jane.
• Educate patients and caregivers on home programs and daily living strategies.
• Communicate with the practice owner regarding patient progress and care needs.
• Travel to patients’ homes within Northern Palm Beach County.

Required qualifications

• Active Florida Occupational Therapist license.
• Experience treating adults with neurological conditions, including Parkinson’s disease and/or stroke.
• Ability to independently evaluate and treat patients with motor, cognitive, and functional deficits.
• Strong clinical judgment and patient safety awareness.
• Familiarity with Medicare outpatient documentation.
• Reliable transportation.

Preferred qualifications

• Mobile outpatient or home-based treatment experience.
• LSVT BIG or PWR! Moves certification.
