Job Summary

We are seeking a dedicated and compassionate Home Health Physical Therapist to join our dynamic healthcare team. In this vital role, you will provide expert physical therapy services directly within patients' homes, promoting recovery, mobility, and independence. Your expertise will help improve quality of life for diverse patient populations.

Responsibilities

• Conduct comprehensive assessments of patients’ physical conditions and develop tailored treatment plans to meet individual needs.
• Deliver effective physiotherapy interventions such as therapeutic exercises, manual therapy, trigger point therapy, and mobility training to facilitate recovery.
• Manage discharge planning processes by coordinating care transitions and ensuring continuity of treatment across settings.
• Document all patient interactions accurately and thoroughly in electronic health records systems following clinical documentation standards.
• Collaborate closely with physicians, nurses, occupational therapists, and other healthcare professionals to optimize patient outcomes.
• Educate patients and their families on home exercise programs, safety precautions, and health maintenance strategies.
• Monitor patient progress regularly, adjusting treatment plans as necessary to achieve optimal rehabilitation results.

Qualifications

• Valid license as a Physical Therapist in Massachusetts
• 1 Year Home Care experience required.

Pay: $70.00 per hour
