A day in the role

  • Turn home visits into everyday victories. Help patients safely navigate stairs, get out of a favorite chair, and walk with confidence.
  • Create a plan with purpose. Assess strength, balance, and mobility, then tailor treatment to each patient's goals.
  • Teach skills that stick. Guide exercises, practice safe movement, and show caregivers how to help between visits.
  • Make the home part of the treatment. Identify fall hazards and recommend equipment or simple changes that support independence.
  • Keep progress moving. Reassess what's working, adjust treatment, and collaborate with PTAs, nurses, and providers.
  • Keep the care team connected. Use mobile technology to document visits, track progress, and share updates.

Give each patient your full attention. Provide one-on-one therapy that builds trust and focuses on their goals, at their pace.

Position purpose

The Home Health Physical Therapist evaluates, plans, provides, and directs skilled physical therapy for patients in their homes. Working under the general supervision of the Director of Clinical Management, the PT helps each patient restore function, move safely, and stay independent at home.

All care follows organizational policy and applicable federal, state, and local regulations, including the Medicare Home Health Conditions of Participation.

Team collaboration

  • Communicate promptly with physicians, nurses, therapists, social workers, and other team members involved in the plan of care.
  • Supervise physical therapist assistants in line with Ohio PT Board rules and Ohio Revised Code requirements.
  • Instruct, supervise, and complete competency evaluations for home health aides as outlined in the Conditions of Participation and agency policy.
  • Take part in case conferences, interdisciplinary team meetings, and QAPI activities.

What you'll bring

Required

  • Doctorate, master's, or bachelor's degree in physical therapy from a program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) or its successor.
  • 1 year experience in licensed field
  • Current Ohio physical therapy license.
  • Valid driver's license, reliable transportation, and auto insurance; must meet agency motor vehicle record (MVR) standards.
  • Strong communication and interpersonal skills, with the ability to speak, read, write, and understand English.
  • Working knowledge of Windows, Microsoft Office, web-based tools, and electronic health records.

Preferred

  • Home health or geriatric experience, including OASIS documentation. New graduates and therapists new to home health are encouraged to apply.
