In this vital role, you will assess individual needs, develop personalized care plans, and deliver evidence-based physiotherapy interventions in the ALF setting.

Duties

• Conduct PT evaluations on easy to use WebPT EMR
• Deliver hands-on therapy sessions in the ALF setting at fabricated wellness centers or homes of the client allowing functional application.
• Manage referral/discharge planning processes by coordinating with multidisciplinary teams to ensure health and wellness at home
• Monitor patient progress regularly, adjusting treatment strategies based on physiology responses and clinical observations.
• Integrate with ALF wellness teams/providers with focus on health/wellness and patient outcomes.

Qualifications

• Valid Arizona licensure or certification as a licensed Physical Therapist
• Strong knowledge of physiology, anatomy, medical terminology, and specialized manual therapies.
• Demonstrated ability to develop effective care plans for vulnerable ALF population.
• Excellent communication skills to collaborate with healthcare teams and educate patients effectively.
• Resiliency and problem solving/time management skills.

Pay: $40.00 - $55.00 per hour
