JOB DESCRIPTION

The RN Care Manager provides compassionate, high-quality nursing care to hospice patients and their families. This role requires strong bedside nursing skills, particularly in symptom management, crisis intervention, and family education. KEY RESPONSIBILITIES:* Manage patient care and provide specialized nursing services to hospice patients.

• Assess and manage patient symptoms and respond to care-related crises.
• Educate patients and families on care plans and symptom management.
• Develop and maintain positive relationships with patients and families.
• Collaborate with the interdisciplinary healthcare team to ensure quality patient care.
• Maintain effective written and verbal communication and demonstrate strong problem-solving and decision-making skills.
• Provide care primarily in patients’ homes, with travel to long-term care facilities, hospitals, and other locations within the service area.

REQUIREMENTS:* Graduate of an accredited nursing school – Required.

• Active Virginia RN license, verified through Nursys – Required.
• Minimum of 2 years of experience in acute care, home health, hospice, medical-surgical, or geriatric nursing – Required.
• Current Virginia driver's license – Required.
• Driving record must meet applicable policy standards – Required.
• BLS (AHA) certification – Required.
