Key Responsibilities:

• Evaluate and treat patients in the comfort of their homes.
• Develop personalized treatment plans to improve mobility and function.
• Educate patients and their families on continued care and exercises.
• Maintain accurate records of patient progress and care.

Qualifications:

• Current state licensure as a Physical Therapist.
• CPR certification
• Previous experience in home health is a plus, but not required.
• Strong communication and organizational skills.
