Job Overview
We are seeking a dedicated and compassionate Occupational Therapist (PRN) to join our dynamic healthcare team. The Occupational Therapist (OT) provides skilled occupational therapy services to patients in their place of residence in accordance with professional practice standards, physician orders, state and federal regulations, and CHAP accreditation requirements.
The OT collaborates with primary care physicians and the interdisciplinary healthcare team and provides education to patients and their families. When nursing services are active, Occupational Therapy functions as a secondary discipline and works collaboratively with the Registered Nurse (RN) case manager to deliver coordinated, patient-centered care.
RESPONSIBILITIES
- Obtains and documents pertinent medical, functional, and psychosocial history from the patient, family, or significant others
- Performs home safety checks and environmental assessments to identify risks and support functional independence
- Conducts occupational therapy evaluations, re-evaluations, and skilled visits and documents services accurately and timely
- Performs physical examinations and reviews relevant body systems as related to occupational performance
- Develops, implements, and revises an individualized Occupational Therapy Plan of Care (POC) and submits it to the physician for review, approval, and orders
- Evaluates patient performance in Activities of Daily Living (ADLs) and Instrumental Activities of Daily Living (IADLs)
- Assists the physician and interdisciplinary team in evaluating the patient’s level of function and rehabilitation potential
- Establishes measurable OT goals based on the patient’s current functional status and potential for improvement
- Provides ongoing functional reassessment and revises the Plan of Care at least every thirty (30) days or as patient condition warrants
- Instruct patients and caregivers in therapeutic activities, self-care techniques, energy conservation, and compensatory strategies to improve independence and safety
- Instructs patients and families in the safe use of assistive and adaptive equipment as ordered
DOCUMENTATION & COMMUNICATION:
- Communicates effectively with patients and families regarding the Plan of Care, goals, and progress toward outcomes
- Maintains ongoing communication with other disciplines involved in the patient’s care
- Communicates with the patient’s physician verbally and/or in writing to obtain OT treatment orders, modifications, or assistive therapy modalities
- Participates in interdisciplinary case conferences to support coordinated care and best practices
- Submits accurate, complete, and timely documentation in accordance with agency policy
- Requests re-authorization for additional visits prior to expiration of third-party payer authorization
- Maintains compliance with CHAP standards, Medicare Conditions of Participation, HIPAA, Medicaid, APD and agency documentation policies
HOME HEALTH AIDE (HHA) COORDINATION AND SUPERVISION
- Develops and implements the Home Health Aide (HHA) Plan of Care when HHA services are ordered and nursing has discharged from the case
- Revises and signs the HHA Plan of Care at the beginning of each certification period when applicable
- Supervises HHAs in accordance with state, federal, and accreditation requirements when OT is the only active discipline and documents supervisory visits
- Reviews billing processes with patients and families and provides education regarding copayments or non-covered services when applicable
