Role Description This is a full-time, on-site Physical Therapist role based in Hays, KS. The Physical Therapist will evaluate patients’ functional status, develop individualized treatment plans, and deliver evidence-based therapeutic interventions across the rehabilitation continuum.
Daily responsibilities include providing direct patient care, educating patients and caregivers on treatment goals and home exercise programs, and collaborating with physicians, nurses, and other rehabilitation professionals to coordinate care.
The role includes documenting assessments, progress notes, and discharge plans in accordance with regulatory and organizational standards, while monitoring outcomes and adjusting interventions as needed. The Physical Therapist will also support quality improvement initiatives, promote patient safety, and contribute to a positive, inclusive care environment.
Qualifications
- Demonstrated expertise in Physical Therapy and Rehabilitation, with the ability to design and implement individualized treatment plans.
- Strong Patient Care skills, including assessment, therapeutic intervention, and patient and caregiver education.
- Ability to integrate principles of Medicine into therapeutic decision-making and interdisciplinary collaboration.
- Experience or readiness to provide Home Care–related guidance, including safe transitions, home exercise programs, and functional adaptations.
- Active or eligible state licensure as a Physical Therapist, and graduation from an accredited Physical Therapy program (DPT or equivalent).
- Effective communication and interpersonal skills, with the ability to work collaboratively in a multidisciplinary team.
- Commitment to inclusive, non-discriminatory care and adherence to ethical and regulatory standards.
- Prior experience in hospital-based or outpatient rehabilitation settings is preferred but not required.
