Job Overview

As a Nurse Practitioner – House Calls serving Westchester County, you’ll assess patients in their homes, develop care plans, and coordinate services with healthcare professionals. You’ll document care accurately, communicate clearly with patients and caregivers, and follow applicable privacy and prescribing requirements.

Responsibilities

• Conduct patient assessments, physical examinations, diagnostic evaluations, and primary care visits; review vital signs and medical records.
• Develop and update care plans, prescribe medications within applicable regulations, and coordinate lab work, and referrals.
• Communicate with patients, caregivers, and healthcare professionals;
• Maintain timely medical documentation in electronic medical record (EMR) or electronic health record (EHR) systems, following Health Insurance Portability and Accountability Act (HIPAA) requirements.
• Your impact: Thorough assessments and coordinated follow-up help patients and caregivers access personalized care at home.

Experience

• Required: Nurse Practitioner qualification and applicable authorization to practice; patient assessment, clinical judgment, communication
• Preferred: Primary care, home care, geriatrics, senior care, assisted living, nursing home, hospice and palliative medicine, acute care, urgent care, emergency medicine, or outpatient experience.
• Familiarity with EMR/EHR systems such as Epic, Athenahealth, Cerner, or eClinicalWorks is helpful; experience with telehealth, triage, medication administration

Pay: $175,000.00 - $225,000.00 per year
