Job Overview

Kings County House Calls is seeking compassionate, dedicated, and experienced Nurse Practitioners (NPs) to provide comprehensive primary care to patients in their homes throughout Queens, NY.

In this role, you will independently manage patient visits within your authorized scope of practice, assess and treat acute and chronic medical conditions, develop individualized treatment plans, and coordinate ongoing healthcare services.

This is a field-based, 1099 independent contractor opportunity offering flexible scheduling, with a minimum commitment of three days per week.

Reliable personal transportation and access to a car are required, as you will travel between patients' homes throughout your assigned service area.

Responsibilities

• Deliver comprehensive primary care to geriatric and homebound patients, managing acute and chronic medical conditions.
• Perform physical examinations, geriatric assessments, and functional and cognitive evaluations.
• Diagnose medical conditions and develop evidence-based, patient-centered treatment plans.
• Prescribe, monitor, and manage medications, including those commonly used in elderly patients, in accordance with applicable prescribing authority.
• Order, review, and interpret laboratory tests, diagnostic imaging, and other clinical studies.
• Monitor patients' responses to treatment, reassess medical conditions, and adjust care plans as clinically indicated.
• Provide preventive care, chronic disease management, routine health maintenance, and patient education.
• Maintain accurate, timely, and comprehensive clinical documentation using electronic medical record (EMR) systems while ensuring compliance with HIPAA and applicable healthcare regulations.
• Coordinate appropriate follow-up care to promote continuity of treatment and reduce avoidable hospitalizations.

Qualifications & Skills

• Active New York State Nurse Practitioner license and appropriate authorization to practice.
• Clinical assessment, diagnostic, and treatment-planning skills.
• Knowledge of primary care, geriatric medicine, and chronic disease management.
• Ability to work independently in a home-based clinical environment.
• Strong communication, organizational, and clinical documentation skills.
• Proficiency with electronic medical record (EMR/EHR) systems.
• Valid driver's license, access to a reliable personal vehicle, and ability to travel between patient homes throughout Queens, NY.

Preferred Qualifications

• Previous experience providing primary care to geriatric or medically complex patients.
• Experience conducting home visits, house calls, or community-based patient care.
