About this role

Do you want a Physical Therapy position where you can focus on the patient instead of rushing from one setting to the next? Iowa Home Care is seeking a Physical Therapist to serve patients throughout the Marshalltown area.

Our home-based therapists help patients work toward greater mobility, function, and independence without having to leave their homes. You’ll have the opportunity to develop one-on-one relationships with patients and caregivers while creating therapy plans that are tailored to the patient's goals and daily environment.

This position is ideal for a PT who enjoys clinical independence, meaningful patient interaction, and being part of a team that truly supports its clinicians.

What You'll Be Responsible For

Your clinical expertise will help patients overcome functional challenges and make progress toward their individual goals. Responsibilities include:

  • Complete physical therapy assessments and evaluations in the patient's home
  • Identify limitations involving strength, range of motion, balance, coordination, gait, and functional mobility
  • Establish individualized treatment goals and plans of care
  • Provide therapeutic exercise and functional training based on patient needs
  • Perform gait, balance, transfer, mobility, and strengthening interventions
  • Teach patients and caregivers techniques that support continued progress between visits
  • Develop appropriate home exercise programs
  • Evaluate the patient's home environment and recommend equipment or modifications when indicated
  • Reassess patients regularly and modify treatment approaches based on their response to therapy
  • Maintain accurate clinical records and complete documentation within required timeframes
  • Report significant changes or concerns to the appropriate members of the care team
  • Coordinate services with nursing, occupational therapy, speech therapy, social work, and case management
  • Encourage safe participation in daily activities and promote the highest practical level of independence
  • Provide care that respects each patient's preferences, abilities, and individual circumstances

  • Licensed to practice as a Physical Therapist in the state of Iowa.
  • Minimum of two years of experience in physical therapy; community/home health experience preferred.
