Physical Therapist (PT) – Home Health | Flexible Schedule | Competitive Pay

FULL-TIME and/or PART-TIME avail, and per-diem

Responsibilities

• Provide Physical Therapy services to older adults in home and community settings
• Evaluate patient mobility, strength, and functional limitations
• Develop and implement individualized treatment plans
• Deliver evidence-based therapeutic interventions to improve movement, balance, and independence
• Maintain accurate Medicare-compliant documentation
• Manage your own schedule and caseload effectively

Ideal Candidate

We’re looking for clinicians who are comfortable working independently in a home health setting and who are organized, motivated, and patient-focused.

Preferred experience:

• Medicare documentation and billing
• Understanding of the 8-minute rule and CPT coding
• Experience managing scheduling and patient caseloads

Requirements

• Active Physical Therapy license in the state(s) of practice or eligibility to apply
• Basic computer and documentation skills
• Current CPR certification

Job Type

Full-time | Part-time | Per Diem opportunities available (W2 and 1099 available!)

Work Setting

• Patient homes within the community
• Community facilities such as Assisted Living Facilities and/or patient group homes
• Home health under Medicare Part B outpatient billing
