Responsibilities

• Assess residents to determine occupational therapy needs and develop individualized treatment plans aligned with goals.
• Implement and monitor therapeutic interventions to improve self-care, mobility, cognition, and functional independence.
• Administer standardized assessments, document progress, and adjust treatment plans as needed.
• Collaborate with an interdisciplinary team to coordinate care and support discharge planning.
• Educate patients and families on home exercise programs, adaptive equipment, safety strategies, and activity modification.
• Maintain thorough and timely documentation in the electronic medical record, ensuring compliance with regulatory standards.

Requirements

• Active Maryland OT license or eligibility to obtain licensure
• Degree in Occupational Therapy from an ACOTE-accredited program (Master’s required; Doctorate acceptable)
• NBCOT certification or eligibility for certification
• Experience in a skilled nursing facility or geriatric rehabilitation setting preferred
• Ability to work in a SNF environment and participate in a travel assignment
• Strong communication and documentation skills; proficient in common electronic medical record systems
