Responsibilities

• Perform comprehensive OT evaluations for residents to determine functional needs and goals
• Create, implement, and monitor individualized treatment plans to improve ADLs and mobility
• Provide therapeutic interventions to optimize independence and safety in daily activities
• Document progress and maintain accurate records in compliance with regulatory and facility requirements
• Collaborate with an interprofessional team to coordinate care, discharge planning, and caregiver education
• Educate residents and families on home exercise programs, safety strategies, and adaptive equipment use

Requirements

• Availability to begin on 10/12/2026 and commit to a 13-week assignment
• Willingness to work a travel assignment in a skilled nursing facility setting
• Ability to deliver 35 guaranteed hours per week
• Flexibility to work the scheduled shift from 08:00 to 16:30
• Proficiency with standard therapy documentation and electronic health records
• Prior experience in geriatric or long-term care settings preferred
