You'll serve as the primary care provider for Medicare and Medicaid patients, helping improve health outcomes through in-home visits, care coordination, and chronic disease management.

Openings in Southeast Phoenix and East Valley, AZ (Tempe, Southeast Scottsdale, and Central East Phoenix) Pay Details $120,000 - $142,000 annually (DOE) Eligible for up to 7.5% Annual Bonus Mileage Reimbursement (IRS rate for travel over 30 miles) Shift: Monday – Friday | 8am-5pm No weekends Benefits for the Field Nurse Practitioner / Physician Assistant: Medical, Dental, and Vision insurance HSA and FSA options 401(k) with employer match Long-Term Disability and Life Insurance Up to 3 weeks PTO 9 paid holidays + 1 floating holiday Company-provided equipment Daily Responsibilities for the Field Nurse Practitioner / Physician Assistant: Conduct 5 to 7 daily in-home and occasional telehealth visits, performing comprehensive patient assessments, diagnosing conditions, prescribing treatments, and managing ongoing care plans Educate patients on chronic disease management, coordinate care with interdisciplinary teams, and maintain accurate documentation in Athena EMR Required Qualifications for the Field Nurse Practitioner / Physician Assistant: Active Arizona NP or PA license 1+ year of primary care, family practice, or outpatient experience Active DEA license Current BLS certification Interested in this opportunity?
