Duties

• Conduct thorough assessments of patients’ physical conditions in home settings, utilizing your expertise in anatomy, physiology, and medical terminology.
• Develop individualized care plans tailored to each patient’s needs, goals, and medical history, ensuring alignment with clinical documentation standards.
• Implement evidence-based treatment strategies such as therapeutic exercises, trigger point therapy, orthopedics interventions, neurology techniques, and gait training to promote patient rehabilitation.
• Perform discharge planning by coordinating with healthcare teams to ensure seamless transitions and continued recovery post-treatment.
• Document all patient interactions accurately using electronic health records (EHR) systems or electronic medical records (EMR), maintaining compliance with clinical documentation standards.
• Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to optimize patient outcomes.
• Educate patients and their families on home exercise programs, safety precautions, and community resources to support ongoing health management.

Skills

• Proven experience practicing in a physical therapy role within home health or outpatient clinics; inpatient or hospital experience is highly valued.
• Strong knowledge of physiotherapy techniques across multiple specialties including orthopedics, geriatrics, pediatrics, neurology, and outpatient rehabilitation centers.
• Excellent client communication skills to motivate patients and explain treatment plans clearly and compassionately.
• Familiarity with outpatient clinic experience and outpatient rehabilitation center protocols enhances service delivery.
• Proficiency in managing clinical documentation standards and working with EMR/ EHR systems for accurate record keeping.
• Ability to perform discharge planning effectively while working with geriatric patients and those requiring home & community care services.
• Knowledge of trigger point therapy and other specialized physiotherapy modalities to address complex pain management needs.

Pay: $60.00 - $80.00 per hour
