Georgetown Living Home Health and Companions is a local, family-owned and operated home health agency providing comprehensive nursing, physical therapy, occupational therapy, and speech therapy services. Our mission has been to provide top notch care for seniors in Georgetown and surrounding areas.

We are rapidly growing/ expanding our services and offer quality/consistent care for orthopedic, neurological and post surgical conditions that require rehabilitation at home for all adult populations including geriatrics.

We seek highly professional, team-oriented clinicians to round out our dynamic team and are looking for PT's all over the Georgetown and Austin area to fill our positions.

Duties

  • Assess patients’ physical conditions by evaluating mobility, strength, flexibility, and functional abilities using comprehensive physiotherapy techniques.
  • Develop and implement individualized treatment plans incorporating therapies to alleviate pain and improve function.
  • Provide hands-on therapy sessions focused on restoring movement, reducing pain, and enhancing overall well-being.
  • Educate patients and their families on physical therapy exercises, injury prevention strategies, and discharge planning to ensure continuity of care.
  • Document patient progress accurately while maintaining compliance with medical terminology standards and healthcare regulations.
  • Utilize objective, standardized tests and measures to document and prove skilled need for care.
  • Supervise PTA's according to agency policy and regulatory requirments.
  • Collaborate with interdisciplinary teams to coordinate care for patients in inpatient, outpatient, geriatrics, pediatrics, orthopedics, or acute care settings.
  • Assist in managing discharge plans by preparing patients for safe transitions to other care environments.

Experience

  • Proven experience in physical therapy within inpatient or outpatient settings; skilled nursing facility experience is highly desirable, and some home health experience is preferred.
  • Specialty skills / certifications will be considered highly desireable.
  • Strong knowledge of anatomy, physiology, medical terminology, and patient care protocols relevant to geriatrics, orthopedics, pediatrics, and acute care populations.
  • Experience working in home health environments or clinics specializing in rehabilitation services is preferred.
  • Ability to develop effective care plans tailored to individual patient needs while demonstrating excellent communication skills.
  • Prior involvement in discharge planning processes ensuring seamless patient transitions across healthcare settings.
