Responsibilities

• Assess patients’ physical conditions through comprehensive evaluations, including physiology and anatomy knowledge, to determine appropriate treatment strategies.
• Design and implement individualized care plans that incorporate therapeutic exercises, manual therapy techniques such as trigger point therapy, and modalities tailored to each patient’s needs.
• Monitor patient progress regularly, adjusting treatment plans accordingly to optimize recovery outcomes.
• Collaborate closely with healthcare professionals—including physicians, nurses, and occupational therapists—to coordinate care and ensure seamless patient rehabilitation experiences.
• Document all clinical interactions accurately in electronic health records (EHR) systems while adhering to clinical documentation standards and medical terminology guidelines.
• Conduct discharge planning by preparing patients for home health or community care settings, ensuring they understand their ongoing therapy routines.
• Work with diverse patient populations—including geriatric, pediatric, neurological, orthopedic, and inpatient cases—delivering expert outpatient rehabilitation services across clinic and outpatient rehab centers.

Skills

• Proven experience practicing in a physical therapy role within outpatient clinics or outpatient rehabilitation centers.
• Strong knowledge of physiology, anatomy, orthopedics, neurology, and geriatrics to inform effective treatment plans.
• Skilled in developing comprehensive care plans that include patient education and discharge planning for home & community care.
• Familiarity with clinical documentation standards and medical terminology to ensure accurate recordkeeping.
• Ability to collaborate effectively with healthcare professionals across disciplines to enhance patient outcomes.
• Experience working with electronic health records (EHR) or electronic medical records (EMR) systems for efficient documentation management.
• Excellent client communication skills to educate patients about their treatment process and motivate ongoing participation in therapy programs.

Pay: $47.70 - $57.45 per hour
